APPROVAL OF CONTRACT CHANGE ORDER #3
TELSTAR INSTRUMEN'T'S, INC.
SEWER FLOW METER REPLACEMENT PROJECT
PROJECT NO. 2003-32
FOR $5,1160.00
WHEREAS, the City received bids and entered into a construction contract with
Telstar Instruments, Inc. on October 12, 2004 to replace a sewer flow meter at the City
pump station; and
WHEREAS, since the commencement of the contracted work, a second sewer
flow meter was authorized for replacement through the project; and
WHEREAS, the two sewer flow meters and corresponding transmitters have
been replaced and are now operational; and
WHEREAS, adjustments to the telemetry system are required.
NOW, THEREFORE BE IT RESOLVED by the City Council of the City of
Rohnert Park that Contract Change Order 93 with Telstar Instruments, Inc. is approved
for the amount of $5,860.00 and the City Manager is authorized to execute the Contract
Change Order.

Dear Shishir,
During our last visit to the Sewer Lift Pump Station and Corp Yard, we found that the existing
SCADA system does not include the flow rate reading for Station #1, and totalizer readings for
both stations. There is no totalizer function in PLC RTU 72A for these flow signals. Therefore a
modification to the PLC program and the Intouch program is needed to have this information
available in the SCADA system. The scope of work follows:
1) Modify the program of PLC RTU72A located in Sewer Lift Station to add the totalizer
function for Station #1 and #2 flows.
2) Modify the SCADA screen "Sewer Lift Station" to add the totalizer reading for both station
#1 and #2 flows, and to fix the existing station #1 flow rate reading problem.
3) Modify the SCADA screen "Current Flow Rate" to fix the existing Sewer Lift Station #1
flow rate reading problem.
4) Modify the SCADA screen "Flow Total" to add "Avg. Daily Flow (Month)" reading and
"Total Flow (Month)" reading for Sewer Lift Station #l, Station 92, and the total of both
Stations.
4) Modify the Excel spread sheet "Monthly Wastewater Report.xls" to have the Sewer Lift
Station 41 and #2 flow rate readings.
5) Provide copy of the updated files on CD.

As part of CO #2, we met with yourself and the Rohnert Park representative to discuss the
requirement for the flow signal and programming. As per your request, we connected the flow
signal from Station 1 to Santa Rosa's telemetry. We did not use the adder because it was
decided to have individual flow signals rather than cumulative. The adder was returned. You
will receive a credit of $540 for the adder and miscellaneous materials. The labor for the adder
in CO #2 was instead used to re- route the cables to Santa Rosa's telemetry box, connect and test.

The programming part of CO #2 was to provide the additional flow signal on the local telemetry
display. This was not completed because we did not have the correct version of the software
required. We have received the correct software from the manufacturer. This was completed on
July 13`h and this closes out the project.
The cost of the additional work for Change Order No. 3 is as follows:

This price does not include tax or freight. We can commence with this at your direction. This quote is valid for
thirty days. This quote is based on information provided to Telstar and may or may not be correct or complete.
Please review this proposal for compliance with the complete and final specifications and drawings before
acceptance. Our terms are due and payable 30 days from date of invoice. Payments must be made on a minimum
of a monthly basis. If payment is not received by the 30th day, a .05% daily service charge (18 -3/4% per annum)
will be charged on all accounts past due. Attorney's fees, court costs and costs of collection will be paid to
prevailing party. Permits and bonding are excluded unless otherwise noted herein. Our standard insurance applies
unless agreed to in writing by Telstar. We accept no responsibility for consequential damages and our standard
warranty applies. Please reference the above stated quote number in all correspondence and purchase orders.
Unless otherwise noted, this quote is based on standard straight time hours and does not include any prevailing wage
rates unless agreed in writing by Telstar. Vehicle expense will be in addition to the price quote, unless specifically
excluded within the body of this quote. Only original or faxed copies of quotes will be honored by Telstar, e- mailed
quotes are not valid. The price quoted herein is for the labor and materials specifically listed within the body of this
quote. Service calls carry a 4 -hour minimum per person. Cancellation charges apply including engineering, labor,
materials, quote and estimating time, markup, % of profit, return goods fees, etc. at the time of written cancellation
notice to Telstar.
