Department heads are not to file any insurance claims directly, with.
any insurance agent or carrier. All insurance claims are to be processed
through the City Manager's office including claims for on the job injuries,
i.e. Workmen"s Compensation claims.
With respect to injuries on the job., Department Heads should solicit
the necessary Workmen''s Compensation forms: from. my off ice,':1.have .t?hemecomh-k
pleted in cooperation with the injured employee, and return the completed
forms to my office for our final approval and forwarding to the insurance
carrier. T
Department Heads are directed to bring all insurance claim items:, i,e.
vandalism, losses due to thefts, fires, injuries, etc. to the attention of
my department within 24 hours after the incident.
